This certificate entitles two adults to receive three days and two nights accommodations in your choice of
10 vacation
destinations. Plan A is the plan you are offered by the company you receive the certificate from. Vacation Incentives USA
only
sends the reservation form to you; you will register your reservation
with the fulfillment center for this offer (not
associated with Vacation Incentives USA).
Plan B is an added bonus for those who wish to travel in less than 60 days, on weekends, holidays or blackout dates at discounted
rates. Please read the following Plan A terms and conditions as you cannot call the fulfillment center office and register Plan A over
the phone or on their website. There is only one way to register the Plan A offer which is to fill out the reservation form you receive
in the Mail and Mail it back to the fulfillment centers office.
If you follow the very simple Terms and Conditions below you will receive notification in the mail from the fulfillment center within
5 to 9 days pre-confirming your requested dates.
Plan A Terms and Conditions
1) Register your certificate/reservation form that you receive from the fulfillment center by filling it out and mailing it within
20 days of the stamped issue date on the back of the form. You will be asked to enclose a check or money order for $29.95 to cover the cost
of all the room taxes. This is your only payment to the fulfillment center for two adults on Plan A.
2) The reservation request MUST be received in our office at least 60 days PRIOR to your first choice arrival date. You must provide an
alternate date that is AT LEAST 45 days after your first choice date. Blackout dates on Plan A are 7 days before and 7 days after holidays or
during local/regional events such as major conventions in Las Vegas, Daytona 500 week in Daytona Beach and Pocono 500 week in the Pocono Mountains.
Forms will not be processed with only 1 arrival date
on Plan A. Myrtle Beach and Daytona Beach areas are
not available on Plan A from June 1st
through August 31st.
3) Arrival days are Monday, Tuesday or Wednesday on Plan A. If you wish to travel on weekends or holidays you must change to plan B. You
and your guest must be at least 21 years of age. If traveling with additional adults/children, additional fees are $25 per night per additional
adult/child (regardless of age) and MUST be paid at the time you submit your arrival dates. Maximum number of people per room is 4.
4) If you do not know your arrival/check-in dates at this time simply register your reservation form as detailed in #1 above. Contact our
office at least 60 days prior to your first choice arrival date and provide 1 alternate date that is at least 45 days after your first choice date.
You may also submit your dates on the fulfillment centers web site as long as you have registered your registration form as detailed in #1 above.
5) Plan A is not valid for groups or persons traveling, arriving or vacationing
together (Plan B is valid for groups). We will make every effort to
fulfill your first choice date request.
6) Customers who wish to add extra nights to their reservation may do so AFTER they have received written confirmation of one of their requested
arrival dates from the fulfillment center office. You can either call their office or use the appropriate form on their web site to add extra nights at
discounted rates.
7) Two Plan A's may not be used in the same city by the same family. If you are using more than one Plan A offer for different cities - arrival
dates must be at least 30 days apart. The same hotels are used for Plan A and Plan B. The hotels can be viewed on the fulfillment centers website by
writing your hotel selection on the reservation form prior to mailing it into their office. Specific hotel requests are not guarantee on Plan A.
Plan B Terms and Conditions
Plan B offers discounted rates when booked at least 10 days in advance. You can view participating hotels, availability and Plan B discounted
rates on the fulfillment centers website or by calling their office. There are NO blackout dates and NO restrictions on Plan B. Discounted rates shown
are for two people occupying one room. Some hotels charge an additional fee for more than two people occupying the room. We will notify you if you
requested hotel charges for additional people. Maximum number of people allowed per room is 4. If you are traveling with more than 4 people, you must
book additional rooms at the Plan B discounted rates.
Plan B rates will vary depending on city, dates and your hotel selection. Plan B is UNLIMITED and never expires. You may allow your friends/family
to book reservations on Plan B at your discounted rates by providing them with your certificate issue date and MC number on the back of your reservation form.
NOTICE
You may use both Plan A and Plan B. If using Plan A, keep your issue date and issue number on the back of the reservation form in a safe place so that you
may use it at another time for Plan B reservations. Plan A may be used ONE TIME - Plan B is UNLIMITED.
There are very few conditions to receive this special promotional offer; the few conditions there are must be strictly enforced in order to honor the
contracts with the participating hotels.
The fulfillment center reserves the right to modify or cancel this promotional offer at any time. They assume no reliability for any verbal claims made by
others in conjunction with the distribution of the reservation form. The fulfillment center reserves the right to book in “surrounding areas” on Plan A during
times of limited space availability. Reservations are subject to the fulfillment centers wholesale space availability and not the hotels rack rate availability.
The fulfillment center is not responsible for lost/late/misdirected mail.
